Through this program, students may receive:

• Comprehensive eye exams

• Glasses, if needed

• Services conveniently provided at school

• No out-of-pocket cost to families!

Please complete, sign, and return the attached Opt-Out Form to your child's school within 3–5 business days. Printed Opt-Out Forms will be available at your student's school front office for your convenience.
